New Patient Instructions for Urology Patients


Upon your arrival, please register with the receptionist. Please take the time to read the following as it will help facilitate your visit.

If you have undergone treatment or testing elsewhere, bring your records with you to this appointment. This may include your doctor’s office notes, previous operative notes, pathology reports, lab report, and X-ray reports. Please be prepared to give a urine specimen at the time of your visit.

If you are coming for a prostate cancer discussion, there is specific information that you will need to bring.

Please complete our patient health history form and bring it and your insurance cards on the day of your appointment. Please be aware that if you have an HMO insurance plan (primary or secondary), you need to contact your primary care physician’s office to obtain the necessary referral authorization prior to your appointment. If we do not have pre-authorization for your visit, you will need to sign a form in which you agree to pay for the services.

If you did not verify your insurance information (name, type of plan, group number, etc.) at the time you made your appointment, please call 314-362-8200 to pre-register. Please have your insurance cards available for this call. This process will expedite your visit.
